New Patient Registration

If you are new to my practice you must complete a new patient online registration.  To access the registration, you must click on

Access Patient Portal

There you will see a prompt – Haven’t Seen Russ Federman? – Register as a new client.

Click on that  and you’ll then taken to a screen which says – Please Activate Your Account.  Once you click on that, you’ll receive an email which says:

We’ve sent you an email containing an account activation link. If you don’t see it in your inbox, it may help to check in your spam folder.

You will then receive an email with a link to your account. 

Once you click on the link in your email, you’ll be taken into the patient portal where you will complete the registration, provide insurance information, and set up credit card payment options (if that will be your preferred mode of payment).  Please be sure to complete all sections.

You can also return to the patient portal in the future to check on future appointments, utilize a secure messaging system to communicate with me, and to make payment on any balances.

Once I receive your registration, I will send you a copy of my Private Practice Information and Guidelines. Please read thoroughly and place your signature in the appropriate locations on the last signature page. Please bring that signed signature page and a copy of your insurance card with you to your first appointment.

Making Online Payment

The Patient Portal can also be used for making online payment via credit/debit card by doing the following:

Click – Log In To Your Account.  When you first registered on my website you provided a username and password.  Enter those when prompted.

Once you have logged in, scroll down to Appointments – Make Payment – and then follow directions for making online payment for any account balance owed.
